If you are unable to print these documents, please contact Human Resources for further instruction so these documents are ready for your signature at your appointment. These documents are required to be completed to comply with our new hire employment process:

Please remember to bring with you to your new hire appointment the following:

  • Completed Documentation
  • Identification (See list of acceptable documents attached to I-9 form),
  • Certifications (RMCBP requires American Heart Association BLS and ACLS)
  • Valid Licensure
  • Voided check for direct deposit
